My JDC Story

 

Although diabetes is ranked as the 7th leading cause of deaths in the United States (CDC, 2021), I have witnessed first-hand the devastating affects this chronic illness had on my family and friends. Unfortunately, today I still watch this disease claim the limbs, organs, and lives of my loved ones and my community. Due to my family history, eventually I began to live with type 2 diabetes. Realizing knowledge is power, coupled with my desire to help end health disparities, I established Jefferson Diabetes Consultants, LLC.

The mission of my organization is to both encourage and equip clients with the tools necessary to properly manage their lives with diabetes by emphasizing the following elements: Education, Motivation, and Empowerment.

Being both a Family Nurse Practitioner and a Certified Diabetes Care & Education Specialist, has afforded me the opportunity to work in various medically underserved communities. Working throughout Chicago and it’s surrounding suburbs, has allowed me the ability to observe how education, motivation, and empowerment positively impacts the lives of people living with diabetes.

Due to the COVID- 19 pandemic, JDC offers diabetes education and consultation, via telehealth, for children and adults diagnosed with type 2 diabetes. This method promotes both the clients and their support systems’ ability to obtain diabetes management services, in the comfort of their homes or on the go. During their sessions, clients are assisted with establishing and achieving SMART goals, in order to increase adherence and self- efficacy.
