Clients have the ability to receive diabetes education and self management support, via telehealth, in the comfort of their homes or on the go. During these sessions, clients aid with the development of their diabetes education and obtain individualized care plans to assist them with making life style changes. Interpretation and sign language services are available per request.

As both a Family Nurse Practitioner and a Certified Diabetes Care & Education Specialist, I have the capability to write referrals and prescribe/adjust diabetes medications to fit the needs of the client.

Clients will receive training on various diabetes devices, such as SMBG monitors (blood glucose monitors), insulin pens, etc. This will allow the client to recognize how various foods, beverages, medications, and/or exercises impact their blood sugar levels; allowing for better diabetes management.

I collaborate with the client’s healthcare team members, while keeping the client at the center of his/her/their care. I also offer consultation services to various healthcare facilities that desire diabetes education and management services for their practices.
